I wouldn't make one for someone else at this point unless they were right here in front of me, especially not for pay. They're really quick and easy to make, but the hardest part is the sizing. This one has a six inch folded over bit in the shoulder, and that's after I cut it down some!
I used this pattern, along with any other patterns I could find by searching for "baby pouch pattern", but really, they all say basically the same thing.
The good thing is that they really are cheap, quick, and easy. I had a little trouble wrapping my brains around the directions at first, but that's really because explaining a simple process in detail makes it complicated :) It probably took me an hour total, including figuring out the directions and the various refittings. If you get two yards of 45" wide fabric, you'll get two pouches out of it. If you get two yards of a lighter weight fabric (so that you could just fold it in half and not have to hem the edges), you could probably even hand sew it within an hour or two if you don't have a machine.
If you do want to buy one, the lady who runs http://www.hotslings.com/ is apparently really good at figuring out sizing.
